If you need to send a COVID-19 screening form to your patients, you can do so quickly and easily. You'll be able to send the form as an SMS message, or an email. This guide will walk you through the steps for both:

Send a COVID-19 screening form via SMS
To send a COVID screening form via SMS, you'll need to generate a one-off form, and then copy/paste that into an SMS message to your patient. This guide walks you through how to create a one-off form, but we'll show you the steps here, too.
On the patient's details page, select the "Forms" tab:
On the next page, press the Add form button:
That will open up a window that asks you to select a form template and an appointment. It's not required that you select an appointment, but if you want this form to be linked to an appointment, then you'll want to make sure to pick a date there:
Press the Create form button, and you'll see the new form in the list of all forms. On the right, you'll see a Copy link button. Pressing that will copy the URL, which you can email, SMS, or paste into a letter (to be sent from Cliniko):

Now, to send that form as an SMS message, you'll want to first press the Copy link button next to the newly-generated form:
Go back to the main patient details page, and press the "Send SMS Message" link, under the patient's mobile number:
In the "Message" field, paste the link you just copied. You can also write a personalised message to the patient:
Hit the Send SMS message button, and you're all set! Your patient will receive that form via SMS. When they fill it out and submit it, it will "live" in the Forms section of their patient file, and you'll be able to view it:

Send a COVID-19 screening form via email
If you'd prefer to send the form as an email, you can do it via Cliniko's Letters function. Follow the steps outlined above, in the SMS section. Then, create a new letter for the patient. You'll be able to paste the form link directly into the letter:
After you create the letter, you'll be able to preview it, and then you can send it (you'll want to use the Email button):
When emailing the form, it's important to make sure that the Format is set to "Send the letter as the content of the email", as this lets the form link be "clickable":
โ๏ธ Note: If you send the letter as a PDF attachment, the link won't be clickable, so the patient wouldn't be able to fill out the form.
