Export a patient form template

Share patient forms with other Cliniko users—they can then add the forms to their account!

As a practitioner, it can be valuable to share resources amongst your colleagues. Cliniko will allow you to export your patient form templates in order to share them with someone else who uses Cliniko!

It's quick and easy to export and share your patient form templates, but you will need to be an administrator on your account to do this one.

We recommend that when you export, that you avoid doing it on Safari. Safari tries to "read" the export file instead of saving it (which ends up resulting in some confusing displays!), so we recommend Chrome!


Export a patient form template

First, head to Settings, and then Patient form templates:

Select the template you'd like to export. On the next page, press the Export button:

That's all there is to it!


To obtain the exported file, navigate to the downloads directory on your computer. If you're on a Mac, this will be in the Finder. If you're on a PC, it will be in your Start menu.

The exported file will have a .json extension and will be titled according to the name of the template that you exported (such as "Patient information.json").

You can share this .json file with your colleague (or they can share theirs with you!), and it can be imported into Cliniko for use as a patient form template.
